The program plan is about women who are currently pregnant getting the prenatal care and education they need in rural Georgia. The plan has a needs assessment, plan, and implementation plan. I have attached an excerpt from the plan. The excerpt includes the needs assessment, the goals and objectives, and the program plan with a logic model.
This piece showcases my ability to:
- Construct a plan based on the needs of a community
- Create goals and objectives based on the needs
- Formulate the plan
The program plan has allowed me to understand the ramifications of creating a plan for people to follow and the difficulties it has. Creating a plan that effects change is hard but when implemented correctly can effect real change in peoples lives.
